Drop the unused pagination slice and the MCP refresh alias with no consumers. Keep fetchServers as the existing refresh path and preserve cached servers on failed reads.

Lock pagination order, page exhaustion, refresh error clearing, and cache retention. Full project tests, root and mobile typechecks, Biome lint/check, docs checks, and both Hermes exports pass.
