Rebased Jason's PR onto main and hardened Codex app-server attachment handling.

- Preserve Codex image attachments as app-server image/localImage inputs
- Cover uploaded Codex image attachments through the session path
- Preserve role context for byte-range-only attachment inputs
- Add coverage for detail passthrough, resumed image-only turns, and non-image omission
