test: harden subprocess fixtures and timeouts for Windows

- Mock codex app-server scripts now exit on stdin close instead of holding
  a setInterval keepalive; Bun's orphan reaper is Linux/macOS-only, so
  abnormal teardown previously left node.exe zombies on Windows.
- Nested bun-test spawns get explicit 60s test timeouts, 55s spawn
  timeouts, and try/finally kills so trees are reaped on outer failure.
- The three JSON-RPC replay flow files now apply the 45s timeout they
  imported but never used; Bun's 5s default was killing tests before the
  harness's own 15s diagnostic wait could fire.
