- Replace deferred() with Promise.withResolvers() in session.ts and
  agentSocket.ts (built-in since ES2024, supported by Bun)
- Consolidate duplicate JWT helpers (base64UrlDecodeToString,
  decodeJwtPayload, isObjectLike, fileExists, readJsonFile) from
  providerStatus.ts — import from codex-auth.ts instead
- Replace 240-line if-chain in startServer.ts with switch statement
- Delete 140-line legacy JSON snapshot import from sessionDb.ts
  (one-time migration that has already run on all installations)
- Remove unused exa-js dependency from package.json (webSearch.ts
  uses raw fetch, not the SDK)
