feat(protocol): stream workflow progress to clients

Adds a `workflow_progress` SessionEvent, projected as
`cowork/session/workflowProgress`, so a run renders as a phase/agent tree
instead of a wall of log lines. Protocol version 7.47 -> 7.48.

Each emission is a full snapshot of the run rather than a delta, so a client
that misses one is not left with a partially-applied tree; the final emission
carries an `outcome`. Threading runs from the tool through RunTurnParams to
`context.emit`, following the `onSessionUsageBudgetUpdated` path.
