The comment composer sticks to the bottom of the conversation, so replying in
a long thread no longer means scrolling to the end first. It is translucent
with a blur rather than an opaque bar, so the thread stays visible underneath.
The merge box moves above it, since a pinned element should be last in the
column.

The approved review card was painted with `bg-primary/10`, and `--primary` is
neutral-50 in dark — a white wash that read as a translucent grey panel rather
than a verdict colour. The fill is dropped so the card is opaque like every
other one, and approval now reads through the border and label using the
existing `--success` token. Changes-requested carried the same wash and gets
the same treatment.

Also fixes an apparent double border on the acknowledge buttons: `Button`
draws a `::before` inner highlight at the default radius, which cut across the
rounded ends of a `rounded-full` pill. `before:rounded-full` makes it follow
the shape, as the slider thumb and radio group already do.
