feat(web): preview tabs that promote on a second open

Adds preview/promote to the workbench tab bar: a single-click open reuses the
preview slot, and opening the same target again pins it. Tab-state logic moves
into tab-state.ts so it can be unit tested without compiling the provider,
matching the split already used for canonical-target.ts.
