Focus-lost was recording undo on every Alt+Tab, muting all apps when a
BetterTrumpet window was focused, and re-applying from the already-changed
volume. Mixer size was overwritten on every device change and when closing
an auto-sized 1-3 device window. Device-change toasts skipped the first real
switch because startup never seeded the current default.

The Linux self-test now covers those cases.
