Remove the automatic mkdir calls for outputDirectory and uploadsDirectory
on server startup. These directories polluted the user's working directory
with folders they didn't ask for.

- Make outputDirectory and uploadsDirectory optional in AgentConfig
- Remove defaults from config/defaults.json
- Fall back to workingDirectory when no custom path is configured
- Only create uploads dir lazily when a file is actually uploaded
  and a custom uploads path is explicitly configured
- Guard all permission checks, command classification, and protocol
  types against undefined values
- Update all 8 system prompt templates to remove separate uploads
  directory references
- Update websocket-protocol.md to reflect optional outputDirectory
