fix(TUI): restore fallback handling for tool descriptors and usage alternate keys

- normalizeToolDescriptor: handle bare string values and empty/missing
  description by falling back to the tool name, matching old behavior
- extractUsageSnapshot: restore alternate provider key names
  (requestTokens, input, prompt, responseTokens, output, completion,
  tokenCount, total) via passthrough record access
