A failed default-plugin bootstrap (marketplace hash mismatch, GitHub
rate limit) used to retry on every workspace server start, re-downloading
the full plugin tree via the GitHub Contents API and exhausting the
unauthenticated rate limit — which then broke user-initiated skill and
plugin installs. Failures now persist to
~/.cowork/config/default-global-skills.failure.json and further attempts
are skipped for 30 minutes (force bypasses; success clears the record).
