Phase 5 of the Bun-native migration. spawnCodexAppServer returns a
StreamingSubprocess; the JSON-RPC client parses NDJSON stdout via
subscribeLines instead of node:readline, writes stdin through the Bun
FileSink, and keeps the SIGTERM-then-SIGKILL stop escalation. Resolver
version/help probes and tar extraction move to execFileCompat (SIGKILL on
probe timeout preserved via the new killSignal option).
