- Remove unreachable block in connectProvider (lines 855-929)
- Remove functions only called from dead block: defaultOauthRunner,
  appendChunkedLines, oauthCredentialCandidates, hasExistingOauthCredentials,
  oauthCredentialSourceCandidates, fileExists, persistOauthCredentials,
  oauthCommandCandidates
- Remove unused OauthCommandRunner type and oauthRunner parameter
- Remove unused CommandRunner type, defaultCommandRunner, and runner
  parameter from getProviderStatuses
- Remove unused spawn and path imports from providerStatus.ts
- ~292 lines removed, zero behavior change
