Production desktop builds dropped all server diagnostics: harness log
and error events only went over the WebSocket, ~/.cowork/logs was
created but never written, and several stores accumulated garbage with
no retention.

- ServerFileLog writes session log/error events to
  ~/.cowork/logs/server-YYYY-MM-DD.log (JSONL, redacted, 0600, 14-day
  retention), on by default; opt out with COWORK_SERVER_FILE_LOGS=0.
- Stale running/pending_init execution states are reconciled to errored
  at boot before any turn can start, so killed sessions stop rendering
  as running and being force-included in thread lists.
- Startup maintenance prunes model stream chunks for sessions untouched
  for 30 days, sweeps snapshot .tmp files leaked by crashed atomic
  writes, and prunes session backup roots.
