Separate ChatGPT account verification from Cowork-managed Codex runtime installation state in provider status and settings UI.

Refresh the runtime status after provider status so the Models page does not show stale install state beside a connected account.

Add regression coverage for missing runtime copy and runtime refresh ordering.
