fix: harden workspace map syscalls and avoid duplicate project instructions

Use readdir with file types plus lstat for roots to avoid following symlink
directories into external paths. Main system prompt keeps profile text only
in {{userProfileInstructions}}; subagents still get ## Project instructions
before the workspace map.
