Protocol version bump from 4.0 to 5.0. Adds:

1. Replay pending ask/approval prompts on reconnect
2. Enriched server_hello with session state on resume
3. Token usage event (turn_usage) from AI SDK response
4. Richer list_tools with name + description objects
5. get_messages query with pagination
6. set_session_title for manual renames
7. Push session_backup_state on connect
8. session_busy with turnId, cause, and outcome context
9. list_sessions to enumerate persisted sessions
10. delete_session to remove old sessions
11. set_config for runtime yolo/maxSteps/subAgentModel changes
12. upload_file with base64 content to uploads directory

All 1257 existing tests pass.
