Codex 0.144.0 spawns a codex-code-mode-host companion from the app-server's
own directory for code-mode tool routing, but the managed installer only
downloaded the app-server binary, so every turn logged
'failed to spawn code-mode host ... (os error 2)' and code-mode tools broke.

- Pin SHA-256 digests for all six codex-code-mode-host 0.144.0 release assets
- Install the host (checksum-verified, atomic rename) before the app-server
  on fresh installs so a failed install never leaves a host-less app-server
- Repair existing installs missing the host, best-effort so an offline
  repair never blocks startup of the already-verified app-server
- Promote the host to current/ with the app-server; skip versions with no
  pinned host checksum (older releases neither ship nor need it)
