- remove duplicate onModelAbort invocation in PI runtime abort path

- preserve ChatGPT-Account-ID for codex-cli PI requests

- plumb includeRawChunks through config and session stream rawPart emission

- add PI runtime telemetry span instrumentation and stream header support

- add/expand regression coverage for runtime headers, abort behavior, and raw chunk gating
