fix: codex app-server critical reliability and integration issues

- Add abortSignal polling during setup RPCs (model/list, thread/start, turn/start)
- Add per-request timeout support to codexAppServerClient and apply 60s timeout to startup RPCs
- Prepend Codex tool boundary note to system prompt instead of appending (stronger attention)
- Fix DelegateRunner to use filterToolsForCodexDynamicBoundary instead of empty tool map for sub-agents
- Improve disconnectProvider to attempt app-server auth revocation with honest UX messaging
- Update tests to match corrected behavior
