Introduce ThreadTitleSource and track titleSource on ThreadRecord. Add normalization and placeholder detection (default vs manual) across persistence and store helpers. Set titleSource on thread creation and local rename, sanitize persisted threads to include a titleSource, and export normalizeThreadTitleSource. Implement logic to selectively adopt non-manual session_info titles (model/heuristic) only when appropriate and only persist when the title actually changes. Update tests to cover sanitization and session_info/title adoption behavior.
