feat: gate built-in task and memories skills behind their features

The bundled task and memories skills are backend services owned by task
mode and advanced memory. They are now only discoverable to agents
(prompt, skill tool, slash commands) while the owning feature is
enabled, and they never appear in the user-facing skill management
catalog, so users cannot disable a service a feature depends on.
User/project skills sharing those names are unaffected.

Also refreshes docs/mcp-guide.md: corrects the mcp-servers.json schema,
replaces legacy method names with the cowork/mcp/* JSON-RPC methods,
and documents the future deferred tool exposure / tool search seam.
