Expand high-risk test coverage

Add focused tests for CLI command behavior, ACME helpers, ACME client flows, deploy behavior, DNS challenge providers, local CA, logging, paths, preflight, secrets, and storage.

Fix DNS exec missing-provider handling so it returns an error instead of panicking.
