Remove the legacy root openkara.db upload path from mutation publication.
Reuse a single durable operation identity through publish, serialize same-
library commits, fail closed when the control DB is degraded, preserve
resumable part files across restart, validate Range responses strictly,
bootstrap via manifest when present, delay GC with durable retry, and
wire the shared network retry policy into production downloads.
