ci: path-aware main push and layered PR/Nightly/Release gates

Stop forcing full CI on every main push; only workflow_dispatch is full.
Document four gate layers. Restore Nightly four-arch separation and dual
macOS installed-app; keep Release on product packaging plus Windows #284
core with primary-arch smokes.
