Close remaining #175 P0/P1 gaps around shared openkara.db semantics:

- Merge ALL Pending/RetryWait change sets (including future RetryWait) and
  inherit max next_attempt_at_ms so the library waits instead of partial freeze
- Prefer CAS-boundary ops in scheduling; reconcile them before any new freeze
- record_completed is one control TX: Complete + Dirty/Clean based on remaining
  non-terminal publish ops and working DB digest match
- Startup repairs Dirty/Publishing after Completed when no outstanding intent
- Mutation prepare/record take explicit library_id (no active re-resolve under lock)
